How  Do  I   Apply  ?

You are doing the right thing in finding a quality program for your child! Some programs accept applications any time. Other programs have Common Application Periods. You will need documents and have to fill out paperwork. The steps below will help you with the application.

1. Choose a program that is the good fit for you and your child.

2. Get all required paperwork together.

3. Find out when to apply.

4. Take paperwork to program site and complete application process.

Child Care Centers:
(Applications accepted any time)

All of the child care centers listed accept Child Care Assistance (CCAP). Contact a center to learn about rates, enrollment and/or application fees, hours of operation, and the ages of children served. Enrollment requirements may differ, so you should CALL a child care center to find out exactly what is needed, but all centers will require the following to apply:

  • Birth Certificate
  • ****Your child MUST be 4 years old by September 30, 2017 to qualify****
  • Current Shot Records
  • Social Security Card
  • One proof of residence (no cell phone bills)
  • Completed application (fill out on site)
  • Income verification; ONE of the following:
    Two (2) consecutive check stubs for EACH PARENT or CAREGIVER IN THE HOUSEHOLD for the current year (within 2 months from the date of filling out this application.)
    IF you work less than 40 hours/week, you must have a signed letter from your employer stating this and your average number of hours worked. 
    OR

    An official letter signed from your employer stating all of the following:
    • Where parent/guardian is employed
    • Hourly rate of pay
    • The average number of hours parent/guardian works per week.
  • SNAP/Food Stamps – must include the child’s name and valid effective dates. This can be a print out from LACAFE.
  • A statement from the Social Security Administration verifying that the child listed on the application is a recipient of SSI benefits, which must be accompanied by two current check stubs.
  • Current foster care placement agreement from DCFS
  • Parents who are homeless or unemployed must submit a letter of support from support source.  (Verification from School system liaison is acceptable)

EarlySteps:
(Applications accepted any time)

EarlySteps is NOT an all day program, but rather provides services to families with infants and toddlers from birth to 36 months of age who have developmental delays or a medical condition likely to result in a developmental delay. Anyone can make an EarlySteps referral, even the family. Children can be referred to EarlySteps by contacting the number below. Once the referral is received, the family will be contacted to begin the eligibility determination process.

      • Current shot record
      • Proof of residence (Examples: Louisiana driver’s license, state-issued ID card, current utility bill, or lease or mortgage statement)
      • Proof of Income (Examples: 2 most recent consecutive check stubs, SNAP card, Medicaid verification sheet, proof of SSI benefit, or official letter from employer stating where you work, your hourly rate of pay and the average number of hours you work per week)
